Long-term use of valganciclovir can cause bone marrow suppression, leading to low levels of white blood cells, red blood cells, and platelets (neutropenia, anemia, thrombocytopenia). Other possible side effects include kidney dysfunction, liver enzyme abnormalities, and an increased risk of infections. Rarely, it may also cause fertility problems and potential carcinogenic effects.
